The blood bank has recently revised the blood request form in favour of a more user-friendly version. The new form will be introduced throughout the province in stages in order to use up stock of the old forms and prevent wastage. Aside from layout changes, the requested information is the same, except for the addition of the requesting doctor’s HPCSA medical practitioner number, which is mandatory and a practice number, if applicable.
